Brokerage Service Policy

This policy describes how Bath & North East Somerset Council and Bath and North East Somerset Clinical Commissioning Group (BaNES CCG) will broker services to support individuals, over 18 years of age, who have been identified as requiring support to discharge/transition from a hospital or community setting into a more appropriate setting that best meets their needs.
