Application Form

An online application form is currently being developed, but the Home Office has provided the following forms in MS Word format:

Supporting Documentation

Your application form must be accompanied by all of the following:

1.  The fee.

2.  A completed Disclosure of Convictions and Declaration form.

This form is a declaration that you have not been convicted of a relevant offence or a foreign offence, or, if you have been convicted of a relevant/foreign offence, gives details of the nature and date of the conviction and any sentence imposed.  A relevant/foreign offence that is spent within the terms of the Rehabilitation of Offenders Act 1974 need not be declared.

3.  One of the following Criminal Records Certificates:

The certificate must be dated no earlier than one calendar month before your application is received by Licensing Services.

Please be aware we are no longer able to accept Disclosure Scotland checks. Any application submitted to us with a Disclosure Scotland check will be rejected.

4.  The original or a certified copy of the Level 2 Award for Personal Licence Holders.

5. Two photographs

These must be:

  • Taken against a light background so that the applicant’s features are distinguishable and contrast against the background.
  • 45 millimetres by 35 millimetres.
  • Full face uncovered and without sunglasses and, unless the applicant wears a head covering due to their religious beliefs, without a head covering.
  • On photographic paper.

One of these photographs must be endorsed with a statement verifying the likeness of the photograph to the applicant by a solicitor, notary, a person of standing in the community or any individual with a professional qualification

6. Right to Work

Anyone that applies for a Personal Licence must provide proof that they are eligible to reside and work in the UK.  Please see the guidance notes attached to the Personal Licence application form for further information about acceptable documentation.

Further Requirements

You will also need to:

  • be aged 18 or over; and
  • have not forfeited a Personal Licence in the past five years; and
  • have obtained a currrent criminal records check; and
  • have obtained the relevant licensing qualification (this requirement can be waived if you are a person of prescribed description under s.120(2)(b) Licensing Act 2003.  This includes, for example, a person operating a premises under a licence granted by the Board of Green Cloth).
