Personal Licences
To make an application you will need to submit the following to B&NES Licensing Services:
- Application form - MS Word Version (77.9 kb)
- Disclosure of convictions and declaration form - MS Word Version (39.5 kb)
Premises Licences
New Application
To make an application you will need to complete the online form at:
As part of your application you will also need to include the following:
- DPS consent form (when the application proposes the sale of alcohol by retail) - MS Word Version (39 kb)
- A plan, drawn to scale, defining the licensed premises for the purpose of the application (see guidance notes).
A copy of your online application will be automatically forwarded to the Responsible Authorities.
If you do not use the online form above you will need to submit copies of your application to B&NES Licensing Services and relevant Responsible Authorites.
You will also need to:
- Display a notice at your premises for 28 consecutive days (this must be printed on PALE BLUE paper) - MS Word Version (32 kb)
- Publish the notice in a local newspaper within 10 working days - MS Word Version (32 kb)
Variation of an Existing Licence
To make an application you will need to submit the following to B&NES Licensing Services and relevant Responsible Authorites:

You will also need to:
- Display a notice at your premises for 28 consecutive days (this must be printed on PALE BLUE paper) - MS Word Version (58 kb).

Minor Variation of an Existing Licence

Transfer of an Existing Licence
To make an application you will need to submit the following to B&NES Licensing Services, the Police and Home Office Immigration:

- Consent from current Licence Holder - MS Word Version (32.5 kb) (this should be completed by the outgoing premises licence holder)
Variation of DPS for an Existing Licence
To make an application you will need to submit the following to B&NES Licensing Services and the Police:
