- To make sure the staff supporting you have accurate, up to date information to help them decide the best possible programme of support for you
- To give a good service, even if the worker helping you is away from work
- To make sure social care services meet the needs of local communities
- To make sure full information is available should you need another type of support, e.g. a specialist service
- To ensure that there is a good basis for looking back and checking on the type and quality of support you received
- To make sure your concerns can be properly looked into if you have a complaint
- To make sure we have an accurate record if we have to review your support
- To check our staff are working well
- To plan new services
We must have information to give you a service.
Do we share your information with other people?
To make sure you receive all the support you need we might share relevant information about you with other organisations. This enables everyone working to support you to have a balanced picture of your needs rather than a snapshot view. It will also save you giving your details over and over again. These organisations may include:
- Health Services
- Young Peoples Service
- Voluntary organisations
- Your school
- Police or Youth Offending Team
We will usually talk to you before information is shared to ensure we act with your consent. We will only share information where it is clearly in your or your child's best interests to do so, and then only as much as is needed.
Sometimes we talk to people such as your school to make sure you receive all the help you need. We will always try to talk to you before we talk to them. We will usually ask your permission, but do not always need to do this. We will normally ask for your consent but there are times when we may need to share your information without your consent, for example:
- where there is a risk of harm or abuse to you or other people
- where a serious crime, such as assault, is being investigated or where it could be prevented
- notification of a birth
- where the courts have made a formal order in relation to a court case
- where there is a legal requirement
Your information should always be shared in a respectful, quiet way and should be kept in a safe and private place. We have a legal duty to maintain the highest level of confidentiality. Generally your records will only be seen by those involved in supporting you. Where we can we don't use your name or address, although a few administrative processes need information that may identify you.
When information is shared, it is passed securely and kept confidential by the people who receive it. It will only be used for the purpose for which it has been shared. We only let people who really need to see your information have it. On a wider scale, we plan services with other organisations outside Social Services. Information shared and used for this does not identify anyone.
Can you see the information we have about you?
Yes, you can. If you wish to see or have a copy of your records write to your last Social Services Office. Please include your full name and address, date of birth, and the service you received from us. If you only want details about one or two issues, please let us know, as we may be able to reply more quickly. You will usually see all your information but very rarely there maybe legal reasons why we cannot share everything with you.
If you are uncertain or unhappy about any of this, please talk to the person working with you.
