Can I specify if I would like a male or female worker?

All workers are allocated to families via the teams allocation meetings, dependent on workload and geographical considerations. All workers have the same basic training and will use the same approach. Families can request a change of worker in extreme circumstances. These will be decided on a case by case basis.

What is a family profile and outcome plan?

Following a whole family assessment, the Keyworker will work with a family to draft a whole family assessment called a family profile. Then, working with the family, the key actions will be written up into the family outcome plan. The plan will have timescales, and the worker will agree who will take responsibility for what and then record it one the plan. They will also record successes, as it is good to keep a record of what has been achieved to date and celebrate success with the family.

How long does your support last?

Normally 6-8 months, but each family is different and so timescales will be decided according to need.

Where will meetings with my keyworker take place and how will you ensure my privacy is maintained?

Connecting Families Team's approach is for most of the visits with your family to take place in your family home.  However, when we invite you to Council premises we will ensure that these are accessible and safe. Where sensitive material is to be discussed with a staff member, or when completing confidential paperwork, meetings will be held away from public areas. If you request your key worker to meet you in a place of your choice, then they will listen to your request and make a decision with you about the setting and how best to keep you and your family's information safe and secure.

What happens to the information I share with my keyworker?

Bath and North East Somerset Council's Children, Young People and Families Team will process the data you have been provided and/or the data provided by you, for the purpose of assessing and providing relevant services to you and your family. We may share your information with other departments and partner agencies for these purposes.
