Exercise Referral
Get yourself a prescription for exercise!

The Passport to Health scheme is Bath & North East Somerset's exercise referral service, enabling healthcare professionals across the authority to refer inactive individuals with a range of other health-related risk factors to a supervised activity and lifestyle change programme. 

The service is commissioned by NHS Bath & North East Somerset and works in partnership with the local leisure providers Aquaterra Leisure and Writhlington Sports Centre.

Your prescription for exercise can offer a range of physical activity pathways including:

All of our physical activity programmes include pre- and post-exercise assessments to ensure each individual receives a tailor-made, safe and appropriate activity programme.  All assessments are free and are provided by trained and qualified exercise referral consultants.

How do I access the Passport to Health Scheme?

Visit your GP or local healthcare professional who can check your eligibility and complete the relevant referral form.
