Moving into a care home is a huge decision for anyone to make. This leaflet is designed to give help and advice on the process for going into a care home if you need help with funding, and general advice for anyone considering entering a care home.
This leaflet is intended to advise people who need to move into a care home permanently. If you need short term or respite care, please see Adult Information Leaflet “Respite Care”.
Local Authority (Social Services) funding may be available to those people who:
- Have a need for care which can only be met in a care home
- Have capital and income which is below certain national limits.
You will need to have a Community Care assessment of your care needs by a member of your local Adult Health and Social Care Community Team. To request an assessment, please contact the Access Team on 01225 396000.
You have a right to an assessment if you are frail, ill or disabled even if you know you will be funding your care yourself. Sometimes alternatives to residential care are available which you may not know about - a social worker can advise you on alternatives which you may wish to consider. Once you have entered a care home it can be difficult to go back to independent living.
From the information gathered in the assessment, a care plan will be drawn up, in discussion with you and (if you wish) your family or other representative, detailing the care you need. The final decision on whether someone is eligible for social services help to live in a care home is made by a panel of managers.
