Adolescent, Young Homeless or Leaving Care
We work with young people aged 13 to 18. These young people can be living with their families, relatives, friends, in supported housing or in foster care. Where the young person is in short term foster care the team works with the young person and their families to support the young person returning home. When a young person is in long term foster care we aim to reduce young people moving foster placements, increase the young person’s participation in education and seek to involve the young person’s parents as far as we are able to in line with the young person’s wishes.
We also work with young people who are still in foster care or who have left foster care aged 16 to 21 or 24 if the young person is continuing in education. When young people reach the age of 16 the team will work with young people to complete a needs assessment and pathway plan. This assessment and plan looks at a young person’s future needs in a number of areas: health, accommodation, education, training, employment, support, money and practical skills. This gives young people an opportunity to start thinking about where they want to be living in the future and what their future goals are. We offer practical, emotional and some financial support to ensure young people have suitable accommodation, contact with their families and previous foster carers, access to education, training and employment and access to health services.
We also have a Young Homelessness Worker who works with young people aged 16 to 18 years who are at risk of homeless or who are homeless.
