All children with a Statement of Special Educational Needs should have a Transition Plan from year 9 (age 13/14) onwards.
A Transition Plan brings together information from people who have worked with your young person so that plans can be made for the transition to adult life.
A number of reports are collected from the teams involved with your young person, your views and your young person’s views are important at the start of the pathway to adulthood.

The Connexions Service offers specialist information, advice and guidance about education, employment and training opportunities, job vacancies and careers. It is a service for all young people aged 13-19 years (25 for those with learning difficulties and/or disabled). The Connexions Personal Adviser allocated to your young person can help with advice, guidance and information about post-school options and other issues that may affect him/ her. The Transition Plan will be reviewed each year until your child leaves school.
