The Health and wellbeing partnership conprises NHS Bath and North east Somerset (NHS banes PCT) and Bath and North East Somerset (banes) Council working together in partnership.

Integrated Assessment Panels

The Integrated Assessment Panels are multi-agency panels made up of representatives of agencies working with young people in the Bath and North East Somerset area. Panels are one of the processes in place to support the integration of services to children and young people at the earliest stage of intervention possible. They are chaired independently, and are committed to the long-term vision of young people having one plan resulting from a holistic assessment of their needs and strengths, which is implemented, managed by a lead professional and reviewed.

The panels operate as assessment panels and welcome the participation of children, young people and families. 

 

0-11 panels

The 0-11 panels have been in operation in their current form since September 2008, and have been through a number of changes since 2006. For further information please see the Integrated Assessment Panel guidance under "Related Documents" (to the right of this page).

11-18 panels
