A Guide for Users & Carers Accessing Mental Health Services

This leaflet provides information on the Mental Health Services provided by Avon and Wiltshire Mental Health Partnership NHS Trust in conjunction with the Local Authority in the Bath & North East Somerset (B&NES) Locality. The locality provides care and support through teams, some based in community settings and others based on hospital wards and day hospitals. The team / ward you get care from is often be determined by where you live as well as your age. Out-patient clinics take place both in community team bases and in hospitals.

The Trust (which provides services across the ex-Avon area and Wiltshire) also provides some specialist mental health services such as eating disorders. Some services are only available during normal office hours while others offer extended hours or 24 hour access.

Intensive Home Treatment Team      

Hillview Lodge, Royal United Hospital, Combe Park, Bath BA1 3NG.(Operates between 10 am – 10 pm 7 days a week).  Where Community Teams feel additional support is required to help prevent admission to hospital, or on discharge from hospital, this Team can become involved in your care for up to 6 weeks at a time

Recovery Team

Referrals received via Care Co-ordinator for people on enhanced ICPA where identified needs are sustained social, functional and/or psychological disability and require sustained effort to engage them to improve their functioning.  They are likely to have severe motivational problems

Active Community Team (ACT) 

(Operates 9 am – 5 pm weekdays and Bank Holidays).  An Assertive Outreach Service to which Community Mental Health Teams can refer service users where additional support is required to maintain contact and improve delivery of effective care and treatment.

Mental Health Liaison Service to the Royal United Hospital (Bath)

(Operates Monday to Friday, plus two weekends each month, between 9 am – 5 pm).  Service provides specialist assessment and interventions for those people admitted to the General Hospital who have mental health needs as well as a physical problem or illness.

(Operates 7 days a week.

Referrals from care co-ordinators to this service. A Supporting People scheme providing practical and emotional support to assist people to maintain their accommodation.  Includes flats for four people within the service, available on a 6 month tenancy.

Referrals from care co-ordinators to this service. Provides a range of practical and emotional support to people in their own homes, working alongside people to help them to maximise their independence, working towards agreed objectives as agreed in the care plan.

Community Support Workers 

Referrals from care co-ordinators to this service. Work primarily in groups, but also one-to-one work, to provide opportunities for social contact, leisure, educational and occupation.

Provides a service to service users and carers from ethnic minority communities. Runs social group as well as providing one-to-one work.

Approved Social Work Service      

Specially trained Mental Health Social Workers who undertake assessments under the Mental Health Act, possibly leading to compulsory hospital admission or Guardianship. Access via Social Services’ Adult Duty desk

The services are provided by a range of professionals, including Nurses, Social Workers, Psychiatrists, Psychologists, Occupational Therapists and others. They offer a wide range of services & treatments, based on individual need including:

  • Medication   
  • Talking Therapies
  • Practical Support     
  • Family Work
  • Occupational Therapy
  • Art Therapy
  • Psychotherapy   
  • & Others        

Leaflets are available on a wide range of specific mental health problems or illnesses and your care co-ordinator can provide you with a leaflet about your condition and about any medication or treatment.

Advice is also available about how you might access additional help in relation to housing problems, police or legal issues, Mental Health Act, work related issues, benefits, training etc. Your care co-ordinator can help you with this.

There are voluntary sector organisations providing services to mental health service users, carers and their families, for both adults and older adults. Information on contacting these services can be obtained from your care co-ordinator

Patient Advice and Liaison Service ( 01225 831717)

The Patient Advice and Liaison Service provides independent advocacy and advice for B&NES service users.

What can you expect from our Services?  

All users who are referred to our service will have their needs assessed. For those who are felt to require on-going care they will have their care outlined in a "care plan". A copy of this care plan will be given to you. Please ask if you have not received a copy. For users whose needs are more complex they will have an "enhanced care plan" and will be told how to access additional support in the event of a crisis. All users will have an identified care co-ordinator.

 If you are a carer who provides regular and substantial of care to a mental health user, you can also have your needs assessed to identify if any additional help can be offered.

How do I get help for myself or someone else?

There is a number of different ways to get in touch with your local services if you are worried about your own mental health or that of someone you know.

  • If you are a current service user - you may wish to contact your care co-ordinator who would usually be available during normal office hours (number above)
  •  If they are unavailable, a member of the same team may be able to offer help
  •  If you have been referred to any of the specialist teams - then they may able to help as well. 
  •  If you are a carer and you are concerned about a service user's safety or your own safety - then you may wish to contact the care co-ordinator or any team they are receiving help from.
  • If you are not already known to the service, then contact one of the Community Mental Health Teams (above) or the Social Services duty desk tel: 01225 396256.

Access to information 24 hours: 

  • For service users and carers who require support outside these hours - please contact your GP
  • In the event of an emergency -your local A&E Department is situated at Royal United Hospital, Bath
  • You can also seek advice from NHS Direct on 0845 4647.

What can I do if I have problems accessing services?

In the first instance, try and discuss your problem with the person or service you are directly dealing with.
