Mental Health

Mental Health Services in Bath & North East Somerset are provided through a joint structure, the Local Authority (Social and Housing Services) together  with Avon & Wiltshire Mental Health Partnership NHS Trust.

These services are known as “secondary services”, provided for people with severe and enduring mental illness, as for most people with mental health problems their needs are met through primary care (i.e. their G.P.). Also within the Service are tertiary services, local specialist mental health services which are accessed by the main community teams. In addition, local services can refer to services which are provided centrally by the Mental Health Trust (such as the eating disorder service) and to other providers elsewhere in the country for very specialist needs.

There are eligibility criteria to decide whether someone is entitled to an assessment and/or a service from the mental health services. These criteria are the Threshold Assessment Grid (known as TAG) and the Local Authority’s Fair Access to Care Services.

Once a person is referred to the mental health service, assessment of needs and any risks, and care planning and reviewing processes, are carried out under the Integrated Care Programme Approach (ICPA). This combines Health’s Care Programme Approach with Social Services’ NHS & Community Care Act responsibilities.

Mental Health Services provide the following:

  • Assessments of need and any risks
  • Assessments under the Mental Health Act
  • Care plans to enable people to live as independently as possible
  • Monitoring and reviews of care plans
  • Practical and emotional support
  • Social activity groups, day care services for people with severe mental illness
  • Support to access training, vocational, employment and leisure opportunities
  • Support to carers
  • Medication
  • Therapeutic interventions
  • Talking Therapies 
  • Family Work·       
  • Occupational Therapy·       
  • Art Therapy·       
  • Psychotherapy   
  • An intensive service targeted at keeping people out of hospital
  • Residential facilities 
  • Hospital in-patient care
  • Day hospitals
  • A liaison service with the Royal United Hospital
  • Support services for people from black and other minority ethnic communities

We also work closely with the independent (voluntary and private) sector.

Most services are provided on the basis that the service user wishes to follow a plan of treatment/support.

Carers are involved wherever possible.

Approved Social Work Service                                     

Operates across Bath and North East Somerset. Specially trained Mental Health Social Workers who undertake assessments under the Mental Health Act, possibly leading to compulsory hospital admission or Guardianship, for those at greatest risk and who are not able to engage with services and treatment in the community, or who are not able to engage with services in the community unless required to do so by legislation.

There are three services within the locality:

Older Adults Mental Health Services

Teams are multi-disciplinary, comprising social workers, community psychiatric nurses, psychiatrists, psychologists, occupational therapists, care workers.

These teams are the gateway to mental health services. They provide assessment, care planning, review and monitoring of care plans, as well as providing or commissioning other services.

St. Martin’s Hospital
Provides in-patient care for people who are acutely ill with dementia. Has assessment and rehabilitation wards.

John Plass Day Hospital
Provides programmes of interventions on a time-limited basis, for people with dementia.

Hillview Lodge Hospital
Provides in-patient care for people who are acutely ill. Two open wards, plus a High Dependency Unit.

Miles House Day Hospital
Provides programmes of interventions on a time-limited basis, plus specialist medication clinics.                                                                                                  

Intensive Home Treatment Team      
Where Community Teams feel additional support is required to help prevent admission to hospital, or on discharge from hospital, this Team can become involved in a person’s care for up to 6 weeks at a time.

Mental Health Liaison Service to the Royal United Hospital, Bath  
Service provides specialist assessment and interventions for those people admitted to the General Hospital who have mental health needs as well as a physical problem or illness

Recovery Team    
Referrals received via Care Co-ordinator for people on enhanced ICPA where identified needs are sustained social, functional and/or psychological disability and require sustained effort to engage them to improve their functioning.  They are likely to have severe motivational problems.

Active Community Team (ACT)                      
An Assertive Outreach Service to which Community Mental Health Teams can refer service users where additional support is required to maintain contact and improve delivery of effective care and treatment.

Sedgemoor Home Link
Referrals from care co-ordinators to this service. A Supporting People scheme providing practical and emotional support to assist people to maintain their accommodation.  Includes flats for four people within the service, available on a 6 month tenancy.

Home Support Service
Referrals from care co-ordinators to this service. Provides a range of practical and emotional support to people in their own homes, working alongside people to help them to maximise their independence, working towards agreed objectives as agreed in the care plan.

Work Development Team
Referrals from care co-ordinators to this service. Assists people with mental illness to keep or gain employment.

Community Support Workers 
Referrals from care co-ordinators to this service. Work primarily in groups, but also one-to-one work, to provide opportunities for social contact, leisure, educational and occupation.

Mosaic
Provides a service to service users and carers from ethnic minority communities. Runs social group as well as providing one-to-one work.
