What do we do?
We are a Home Care team dedicated to providing a ‘re-enablement’
service to people who need support to regain confidence and daily
living skills, for example, following a spell in hospital.
Referrals will be made to the team following a Community Care
Assessment.
This service will last up to a maximum of 6 weeks,
during this time, our Re-enablement Support Workers will assisst
with personal care and provide support, whilst
encouraging or enabling the service user to regain
independence within their own home.
The Re-enablement Support worker will constantly monitor the
improvements of the service user and adjust the care provisions as
and when required. It is anticipated that most Service users
requirements will decrease during their time with the team as they
re-adjust and develop in confidence, the team will respond
accordingly.
After 4 weeks a review of the service will be arranged to
discuss the on going needs of the service user. This sevice
is not intended to provide respite care.
Who is eligible for the service?
The service is open to people who:
- Are aged 18 and over
- Live in the Bath & North East Somerset area
- Have had a Community Care Assessment by a Social Worker or Care
Service Co-ordinator and is deemed eligible under the Council
eligibility criteria.
- Require at least 5 visits per week, but who are able to manage
on their own alone between visits
- Have given their consent to have the service and who have the
potential and willingness to improve
Who is not eligible?
The service is not open to people who:
- Require less than 5 hours’ home care per week
- Require palliative care/ or are eligible for CHC funding
- Have a current care plan for a home care service that needs
re-starting
- Have a Carer who requires respite
How do I make a referral?
We can accept referrals from:
- All Hospital Teams
- Adult Care Teams
- Community Mental Health Team for Older People
- Adult Duty Desk
Referrals are made to the team on the understanding that this is
the wish of the service user and that there is an achievable
goal.

Who’s who in the Intake & Re-enablement Team?
The people who actually provide the care are called
Re-enablement Support Workers (RSWs). All our RSWs follow an
induction programme when first employed which includes training in
First Aid and Manual Handling. Many RSWs have also completed
National Vocational Qualification (NVQ) training at Level 2
or 3.
The people who organise the care on a daily basis are called
Co-ordinators. Bev Bishop and Sue Breakah are the
Co-ordinators for this service and have achieved NVQ Level 3 or are
in the process of doing so.
The Intake & Re-enablement Team’s Locality Manager is Dawn
Cronin. She is responsible for the overall running of the
service and manages the team on a day-to-day basis.
Sue Kennedy is the Administrative Assistant who will provide
office support and answer telephone calls.
