Information for Patients and Carers Who Live in Bath & North East Somerset

ARE YOU A PATIENT?

Do you need help with:

Washing and dressing

  • Household tasks
  • Meal preparation
  • Supported living
  • Residential care
  • Access to other services such as housing and benefits advice

ARE YOU A CARER?

If you are looking after a relative or a friend do you need:
  • Help providing care
  • A break from caring
  • Support to continue caring
  • Advice about continuing at work
  • Access to other services such as Carer support groups and benefits advice

THE HOSPITAL SOCIAL WORK TEAM

When someone is planning to leave hospital they may need support or advice from Social Services

Bath and North East Somerset Social Services have an Adult Care Team based at the Royal United Hospital who are able to provide support and advice to anyone planning to leave hospital.

Before providing or arranging any help we always carry out an assessment of needs.

If you are a patient or a carer you are entitled to request an assessment.

Following an assessment we will provide information about services which are available to meet your needs.
