THE HOSPITAL SOCIAL WORK TEAM
When someone is planning to leave hospital they may need support
or advice from Social Services
Bath and North East Somerset Social Services have an Adult Care
Team based at the Royal United Hospital who are able to
provide support and advice to anyone planning to leave
hospital.
Before providing or arranging any help we always carry out an
assessment of needs.
If you are a patient or a carer you are entitled to request an
assessment.
Following an assessment we will provide information about
services which are available to meet your needs.
