An Introduction To A Community Team

What is a Community Learning Difficulties Team (CLDT)?

A CLDT is a group of specialist professional staff from Bath & North East Somerset Primary Care Trust and B&NES Social Services Department who provide assessment, support, advice and therapeutic intervention to people who have learning difficulties;

Who Does the team work with?

  • Service users and their families
  • GP's and Health Centres
  • Health services including hospitals
  • local authority hostels and residential units
  • Nursing and residential homes
  • Housing & Social Services Department
  • Resource and activity centres
  • voluntary organisations
  • adult education colleges

How does the Team operate?

We have an open referral system to clients living in our area.

To assist people who have learning difficulties to achieve their full potential and enjoy an integrated, varied and fulfilled life.

  • Psychologists
  • Community nurses
  • Community support workers
  • Psychiatrist
  • Occupational therapist
  • Physiotherapist
  • Speech and language therapists
  • Behaviour nurse pracitioners
  • Hearing therapist
  • Healthcare/community support workers
  • Social workers (attached to the team to respond to referrals received under the NHS & Community Care Act (1993 procedures)

The clinicians are supported by a locality manager, administrators and an adim. team.

In addition the team has access to the service of:

  • Dietician
  • Forensic psychologist
