Adult Disability -  Community Learning Difficulties Team

The team works with people who have a learning difficulty.

We hear about people who need our help from the Adult Care Duty Desk. Their address is:

The Children’s Disability Team tell us about young people who are about to become adults.  Their address is:

Forbes Fraser Unit, Royal United Hospital, Bath.

The team works to promote the independence, rights and choices of Adults with Learning Difficulties and to make sure they are included in everything.

People in the team work with service users and their carers, to find out what their needs are. The service user and their carer take part in planning a package of care.  This means finding out what a person needs.  We will make sure that we provide the services that are needed or find the right people to do it.

The team has:

  • 6 Social Workers
  • 2 Assessment and Reviewing Officers and
  • 2 administration staff.

They help the team to do their job.

Every day, the team is managed by the Assistant Team Manager. 

The team works with other people.  They work with people from the Health Community Learning Difficulties Team and share offices with them.  They also work with other people who provide services that we need.
