An introduction to the Community Team
What is a Community Learning Difficulties Team (CLDT)?
A CLDT is a group of specialist professional staff from Bath
& North East Somerset Primary Care Trust. Avon &
Wiltshire Mental Health Partnership NHS Trust and B&NES Social
Services Department. We provide assessment support, advice,
therapeutic intervention and training to adults who have learning
difficulties. We also provide training to parents/carers and
others, as well as more formal training for other service
providers.
Who does the Team work with?
We work in partnership with:
- Service users and their families
- GPs and Health Centres
- Health services including hospital
- Nursing homes
- Residential homes
- Housing & Social Services Department
- Day services
- Voluntary organisations
- Adult education colleges
- General public
- Service providers, e.g. Mencap, New Era
- Employers
How to get help from us
Anyone can refer a person with learning difficulties who lives
in the B&NES area to us, e.g. parent\carer\the person
themselves\G.P.\ neighbour, other professionals, etc.

Our Aim Is...
to assist people who have learning difficulties to achieve
their full potential and enjoy an integrated, varied and
fulfilled life.
This includes help and advice now, and planning for the
future.
Who is in the Team?
The following professions are represented:

- Speech and language therapy
- Behaviour nursing
- Hearing therapy
- Healthcare support
- Social work
- Administrative support
We are a multi-agency team, supported and co-ordinated by a
Health Locality Manager and Social Services Team Manager.
In addition the team has access to the services of:
- Dietician
- Forensic psychologist\community nurse
- Sexual health worker
