Referrals:
Referrals to the team come via the
Adult Care Duty Desk based at Lewis House, Manvers Street,
Bath. Tel. 01225 477000.
We also receive referrals about young people approaching
adulthood from the Children’s Disability Team based at Forbes
Fraser Unit at the Royal United Hospital Bath.
The Team:
The team works to promote independence, rights and choices of
Adults with Learning Difficulties.
Team members work with service users and their carers in
identifying their needs. They then involve them in planning their
package of care and providing services according to assessed
needs.

The team works closely with the Health Community Learning
Difficulties Team staff in a multi-disciplinary setting, which is
based on the two same locations as above, and with a range of
Service Providers.
