Using the online referral form to request a service or information from Social Services

Who should use the form?

Bath and North East Somerset Social Services provides a range of services to people who:-

  • are disabled (including people with a sensory impairment)
  • are frail and elderly
  • have an enduring illness
  • have a learning difficulty
  • have mental health problems
  • are in some other way vulnerable
  • or who care for someone who is disabled

The form can be filled in by any disabled person or by someone else on their behalf. The form can also be used by a carer who is caring for a disabled person.

When to use it?

You can use this form at any time to request a service or an assessment of your needs. However if your need is urgent you should phone us on (01225) 477000 (or ask someone to phone on your behalf) and ask for the Adult Care Duty Desk which is open from 8.30 am - 5.00 pm (Monday to Thursday) and 8.30am - 4.30 pm (Friday).

What will happen next?

Somebody from Social Services will telephone or e-mail you to collect further information within three working days. We need additional information in order to decide:-

  • The most appropriate member of staff to deal with your request
  • The priority for dealing with your request (we prioritise all referrals to ensure that those people who are most at risk are seen urgently)

If you are simply requesting information and advice, it may be possible to deal with this immediately over the phone (or by e-mail).

We provide very few services without first making a home visit to discuss your needs and the services which may be available to help you. If your need for an assessment is fairly urgent, somebody from Social Services will phone you to make an appointment within one working week. Otherwise you will be sent a letter or e-mail within five days telling you how long you will have to wait for a visit.

Does everyone who requests a service receive one?

Unfortunately, our resources are limited and not everyone who requests a service will receive one. Social Services publish eligibility criteria which set out the circumstances in which we will definitely provide a service, circumstances where we provide a service when resources allow and circumstances where we are regrettably unable to provide a service. This will be explained to you if you have a visit to discuss your needs.

Even if we are unable to provide a service we endeavour to provide advice and information which can assist you in getting help.
