Eligibility Criteria - Fair Access to Care Services

1.  RISK TO INDEPENDENCE – HEALTH & SAFETY – the risk to the health (including mental and physical health) of an individual and risks to their safety and the safety of others caused by themselves or other people.

 2.  RISK TO INDEPENDENCE – CHOICE – the control a person has over their life and the            choices they are able to make

else who is able to assist.

 RISK TO INDEPENDENCE – MANAGEMENT OF DAILY ROUTINES – the ability of a person to look after    their own personal care, domestic needs and other daily routines

3.a

Needs intensive daily care, with personal care needs  e.g. transfers, washing, dressing, using the toilet. This can include help to carer.

Unable to manage any domestic tasks and there is no family member, carer or friend to help

Without the provision of aids or adaptations (could include access to and within a property)  the disabled person will be unable to carry out any of the above tasks

Carer is no longer able or willing to provide support and assistance with above (includes young carers).

SUBSTANTIAL(risk is present now or likely to arise within 3 months)

There is, or will be, an inability to carry out the majority of personal care, and/or domestic routines.

3.b

Requires a significant amount of help with personal care needs (e.g. transfers, washing, dressing, using the toilet).  This can include help to the carer.  A minimum of 3 hours assistance per week.

Has unpredictable and/or increasing needs and without assistance the situation would deteriorate rapidly

Needs time limited support from qualified staff to regain their independence

Without provision of aids or adaptations the disabled person will be unable to carry out most of the above tasks (could include access to and within a property and key domestic routines) 

Needs help and supervision to manage most domestic tasks and there is no family member, carer, friend to help. Min. of 2 hours per  week

Carer provides 6 hrs or more of personal care per week and without additional support being provided to the person they care for, the caring situation will break down.

Disabled person is unable to heat up or organise a hot meal safely and there is no family member, carer, or  friend to help

Carer is providing substantial and regular care and requires a carers service or carers break to enable them to go on caring.  

MODERATE(risk is present now or is likely to arise in the next 6months)

There is, or will be, an inability to carry out several personal care and/or daily routines

 

Unable to carry out – a strip wash or bath,  and/ or hygienic cleaning, on a regular basis and this  may pose a threat to their physical wellbeing

LOW (risk is present now or is likely to arise in the next 12 months)

There is, or will be, an inability to carry out one or two personal care and/or daily routine

 

Has difficulty maintaining their house to preferred  standard of cleanliness

Assistance with some domestic tasks would improve a person’s quality of life

Has difficulty bathing but is able to have a strip wash

Aids may help with some tasks but these are readily available from specialist shops etc. Bathing aids and rails would assist a person but they are able to manage a strip wash


RISK TO INDEPENDENCE – INVOLVEMENT IN FAMILY AND COMMUNITY LIFE – involvement in work, education, family life and social networks, and community activities (including the responsibilities of parenting and caring).

Vital support systems and relationships are, or could be, at great risk

4.b

Needs specialist support from SS to maintain social relationships outside the home – contact at least 3 times  per week

Carer requires immediate break in order to sustain caring role.

 

Vital family and other social roles and responsibilities can not or will not be undertaken

4.c

Without considerable assistance the disabled person cannot provide care to or supervise their child or children.

Without  professional support, vital relationships will breakdown.

SUBSTANTIAL(risk is present now or likely to arise within 3 months)

Involvement in many aspects of work, education or learning can not or will not be sustained.

4.d

Requires the specialist support and co-ordination of SS  (minimum involvement of one contact visit per week) to  sustain their involvement in work/training and education (this includes personal living skills).

 

The majority of social support systems and relationships can not or will not be sustained

4.e

Requires specialist support  from SS to develop and sustain relationships outside the home – minimum contact  -    once a week

 

The majority of family and other social roles and responsibilities can not or will not be undertaken

4.f

Needs assistance to carry out many aspects of their  parenting role and cannot assume sole charge for sustained periods of time e.g. when a partner is away from home

·         Key relationships have broken down and this is having a detrimental effect on an individual’s mental health

Carer is under stress and may not be able to sustain their caring role as well as other family roles.

MODERATE (risk is present now or is likely to arise in the next 6months

Involvement in several aspects of work, education or learning can not or will not be sustained.

 

Can maintain involvement  in work training/education with help from other statutory agencies and voluntary organisations but some assistance would enhance their opportunities

 

Several social support systems and relationships can not or will not be sustained

 

Needs some assistance and/or encouragement to participate in social and community activities

 

Several family and other social roles and responsibilities can not or will not be undertaken.

 

Some aspects of the parenting role such as taking children out or to school cannot be undertaken.

LOW (risk is present now or is likely to arise in the next 12 months

Involvement in one or two aspects of work, education or learning can not or will not be sustained.

 

Is able to maintain their involvement in work/training/ education with help and support from other statutory and voluntary organisations

 

One or two support systems and relationships are, or could be, at risk of not being sustained.

 

Experiences diminishing social activity which has an  impact on quality of life

 

One or two family and other social roles and responsibilities can not or will not be undertaken

 

Can take an active part in family life but additional support could enhance their role.
